Healthcare in 3D Terms and Phrases

Patient Centered Medical Home: Is a methodology to provide primary comprehensive patient centered care, where a single physician or “medical home” is responsible for coordinating the patients care throughout the patient journey. In the context of HCn3D it is the observation and influence the medical home is accountable for across all network entities providing care.
